Permanent employees on-boarding
Permanent employees are required to complete the on-boarding forms listed below and submit these forms to the CSIR Human Capital department prior to commencement of employment at the CSIR:
- Application for a CSIR identification card
- Application to perform private/professional extra work
- Application to serve as Director of a company etc.
- Consent to receive CSIR information via SMS
- Department of Labour declaration
- Declaration of conflict of interest
- Medical questionnaire
- Personal details
- Pledge of secrecy, ICT policy and ICT acceptable use standard
- PoPIA Section 18 Notification

- Medical scheme booklet 2023 (refer to sections 1 – 4.5)
- Gap Cover – Application Form 2024
- Gap Cover – Brochure 2024
- CSIR Pension Fund and Employee Benefits brochure
- CSIR Pension Fund Beneficiary Nomination form 2024
- CSIR Life cover Beneficiary Nomination form 2024
- CSIR Funeral Scheme Nomination form 2024
Permanent employees are also required to familiarise themselves with the following:
- CSIR Ethics Statement and Code of Conduct
- CSIR Conditions of Service
- CSIR ICT Policy
- ICT Acceptable Use Standard
